New patients attending multidisciplinary teams for oesophagogastric cancer at regional NHS Scotland facilities from October 2019 to September 2020 constituted the cohort for this retrospective study. Diffuse large B-cell lymphoma (DLBCL) is the prevailing type of B-cell non-Hodgkin lymphoma (B-NHL) found in adult populations. Gene expression profiling (GEP) is employed to classify these lymphomas into germinal center B-cell (GCB) and activated B-cell (ABC) lymphoma types. Among the novel subtypes of large B-cell lymphoma, identified through recent studies based on genetic and molecular alterations, is large B-cell lymphoma with IRF4 rearrangement (LBCL-IRF4). To comprehensively characterize 30 cases of LBCLs in adult patients situated in Waldeyer's ring and to pinpoint the LBCL-IRF4 subtype, we employed fluorescence in situ hybridization (FISH), genomic expression profiling (GEP), and next-generation sequencing (NGS). FISH examinations displayed IRF4 breaks in 2 samples out of 30 (6.7%), BCL2 breaks in 6 out of 30 cases (200%), and IGH breaks in 13 cases (44.8%) out of 29 total cases analyzed. Fourteen cases were each categorized by GEP as either GCB or ABC subtypes, while 2 cases remained unclassified; this classification aligned with the immunohistochemistry (IHC) results in 25 out of 30 instances (83.3%). Based on GEP analysis, a subgroup was identified; group 1 contained 14 GCB cases, with the most prevalent BCL2 and EZH2 mutations observed in 6 of these cases (42.8%). Among the cases in Group 2, 14 were classified as ABC; the mutations CD79B and MYD88 were most frequently observed, appearing in 5 of the 14 patients (35.7% incidence). Two unclassifiable cases, marked by an absence of molecular patterns, were part of Group 3.
