The study cohort encompassed patients with cirrhosis presenting with AVB across 34 university hospitals in 30 cities during the period from February 2013 to May 2020, undergoing endoscopy within 24 hours. Patients were grouped into two categories according to the timing of their endoscopy procedures: the urgent endoscopy group, which received endoscopy within six hours of their admission, and the early endoscopy group, which underwent endoscopy between six and twenty-four hours post-admission. Risk factors for treatment failure were determined using a multivariable analytical approach. The central metric of success, or failure, was the five-day treatment failure rate. Secondary outcome measures encompassed deaths occurring during hospitalization, the need for intensive care unit services, and the length of time spent in the hospital. An analysis using propensity score matching was carried out. Our analysis additionally involved a comparison of 5-day treatment failure rates and in-hospital death tolls in patient groups categorized by endoscopy timing: those having the procedure in less than 12 hours and those having it between 12 and 24 hours.
A total of 3319 patients participated, categorized as 2383 in the urgent endoscopy group and 936 in the early endoscopy group. After adjusting for confounders using propensity score matching and multivariable analysis, Child-Pugh class was found to be an independent predictor of 5-day treatment failure (hazard ratio 1.61, 95% confidence interval 1.09-2.37). The urgent endoscopy group experienced a 30% incidence of 5-day treatment failure, a similar rate to the 29% observed in the early group (p = 0.90). Urgent endoscopy procedures resulted in a 19% in-hospital mortality rate, contrasting with the 12% mortality rate observed in the early endoscopy group (p = 0.026). Endoscopy procedures categorized as urgent were associated with a 182% higher incidence of intensive care unit requirement, surpassing the 214% observed in the early endoscopy group (p = 0.11). In the urgent endoscopy group, the average hospital stay was 179 days, contrasting with 129 days in the early endoscopy group, a statistically significant difference (p < 0.005). Within the <12-hour treatment group, 23% experienced treatment failure after five days, while 22% in the 12-24 hour group failed (p = 0.085). A significant difference (p < 0.05) was observed in in-hospital mortality rates, being 22% in the under-12-hour group and 5% in the 12-24-hour group.

The study sought to quantify the occurrence and identify the specific types of bullying present in orthopaedic surgical settings in the United States.
